If you have moved to a different address within the Camp Hill Borough, please print and complete the form below and submit the form and TWO proofs of residency (see list on the form) to the Registrar in the Administration Office. Forms and documentation must be submitted in person. The Administration Office is located at 2627 Chestnut Street.
Student & Parent Information Changes
Phone numbers - changes to parent/guardian and emergency contact phone numbers must be made by the parent/guardian in the SIS Parent Portal. Parents/guardians must also update their phone numbers in the Blackboard Alert Portal.
Email addresses - changes to parent/guardian and emergency contact email addresses must be made by the parent/guardian in the SIS Parent Portal. Parents/guardians must also update their email addresses in the Blackboard Alert Portal.
Name changes - parents/guardians must bring proof of a name change for students and/or parents/guardians to the Registrar in the Administration Building.
If you are moving outside the Camp Hill School District, you will need to withdraw your child(ren) from school. During the school year and the summer, please contact the administrative assistant at your child's school to begin the process. Upon withdrawal, a student is to return all of his/her textbooks, library books, athletic uniforms, and any other school-provided supplies and equipment.
